RESUMO

Purpose of the study improving the diagnosis of precancerous diseases and cancer of the oral mucosa using fluorescent immunocytochemical studies by direct immunofluorescence. A clinical laboratory examination of 111 patients was carried out: 46 patients with squamous cell carcinoma of the oral mucosa, 35 people with precancerous lesions (17 leukoplakia, 18 - oral lichen planus) and 30 healthy people. All patients underwent a traditional cytological examination and an additional immunocytochemical examination by direct immunofluorescence, the expression levels of tumor markers P53, P16 and Ki67 were determined. The data were compared with the results of histological analysis. As a result of the study, it was revealed that in patients with cancer, the expression of oncomarker P53 was four times higher than in patients with precancerous pathology. In 6.52% of cases, co-expression of markers Ki67 and P16 was found. Thus, the advantages of fluorescent immunocytochemical diagnostics were the absence of invasive traumatic intake of the biomaterial in the patient, reduction in the timing of obtaining the result, high sensitivity, and the possibility of remote evaluation of the results. Therefore, that increases the accessibility of the method, and the possibility of using this method for a screening study of population.

Orthopedic treatment is of particular difficulty in the complex therapy and rehabilitation of patients with precancerous diseases of the oral mucosa. In this regard, in patients with chronic inflammatory and destructive lesions of the oral mucosa, it is necessary to control changes in the oral cavity, taking into account clinical and laboratory data. Oral fluid is most often used as a diagnostic material in dental research, the composition and properties of which reflect functional changes in the body. However, the studies that substantiate rational orthopedic treatment in such patients are extremely insufficient: there are no data on the effect of orthopedic structures on the primary metabolism in the oral cavity in patients with precancerous diseases. In this regard, the purpose of this work was to study the biochemical parameters of the oral fluid in patients using orthopedic structures to identify the prognostic factors of precancerous conditions. In the oral fluid, the levels of total protein, total cholesterol, triglycerides, and lactate were determined. Free radical oxidation was assessed by the intensity of induced biochemiluminescence and by the levels of lipid peroxidation products. In the oral fluid of patients with mucosal precancer compared with patients in the control group, a significantly higher intensity of free radical oxidation was revealed with a corresponding decrease in antioxidant activity. The level of the end products of lipid peroxidation - Schiff's bases - was statistically significantly increased. Among the metabolites of the oral fluid, there was a decrease in protein levels and an increase in cholesterol and lactate levels. At the same time, a positive correlation was established between the indicators of free radical oxidation and lactate, free radical oxidation and cholesterol, but negative with the levels of protein and triglycerides. Regarding the antioxidant system, there was a negative correlation between the tg (-2α) value and the lactate level and positive with the triglyceride level, as well as a positive correlation between the Z parameter and the total protein level. The obtained changes in the biochemical parameters of the oral fluid can be used to confirm the diagnosis of precancer of the oral mucosa in patients with orthopedic structures.

Research objective was assessment of a possibility of primary diagnosis of viruses Epstein-Burr, a cytomegalovirus, a virus of herpes of the 6th type in oral liquid and also influences of herpes infection on development and the clinical course of precancer diseases of the oral mucosa and the red border of lips (RBL). Profound clinic-immunological examination is conducted to 60 patients: the first group are have made 20 patients with an erosive and ulcer form of the lichen planus, the second - 20 people with an erosive form of a leukoplakia, a third - 20 people without diseases of the oral mucosa. As a result of work detection herpes infection is revealed high (90%). The combined infection with a virus Epstein-Burr and a virus of herpes of the 6th type was observed more than at a half of patients. The imbalance of factors of local immunity of the oral cavity in the form of increase in the IgG profile, decrease in concentration of IGA, lysozyme, and increase in an indicator of Ksb three times in comparison with norm and also substantial increase of level of pro-inflammatory cytokin IL 1ß and FNO-α is found in patients with precancer diseases of the oral mucosa. Characteristic clinical feature of the precancer diseases associated with latent herpes infection is the long, recidivous current, persistent to traditional therapy. The research of oral liquid and blood on herpes viruses and consultation of the infectiologist is recommended to all patients with precancer diseases.

RESUMO

The article presents findings allowing estimating effect of local application of polioxidonium and yantum verde in 101 children aged 12-17 with chronic catarrhal gingivitis and chronic gastroduodenitis. Statistically significant PMA indeх decrease (40.1±2.3% till 1.4±0.6% (р<0,001)) proved the above mentioned therapy scheme to be highly effective for treatment of chronic catarrhal gingivitis in children with chronic gastroduodenitis.

RESUMO

The study revealed higher frequency and intensity of pain after root canal filling with paste Foredent or Zn-eugenol paste in patients with bronchial asthma and showed significant change in the level of sIgA in saliva fluid depending on the filling material and reflecting the effect of root pastes on the local immunity of the oral cavity.

RESUMO

Results of laboratory investigations of the estimation of peroxide oxidation of lipids and antioxidant protection in the oral fluid of the patients with chronic generalizating parodontitis and arterial hypertension with using mexidol. It was shown that the normalization of parameters of the primary and second products of peroxidation with mexidol action, antioxidant protection of oral liquid increased that was favorably reflected in structures and functions of cell.

RESUMO

Histamine releasing and immunomodulating activity of the following dental restorative materials (DRM) - Prizmafil, Filtek Z250, XRV Herculite Prodigy, Glasiosite, Te-Econol, Valux Plus, Polofil Supra were studied. It was shown that DRM under study as a rule did not possess the ability to release histamine (H) from human blood basophile (BB) excluding Filtek Z250 which release H from BB in patients with allergy and sound donors. The studied DRM implanted under mouse skin were able to modulate immune response to the allergen, at that some of them increased antibodies of IgE-class forming and other suppressed immune response caused by IgG-antibody forming. Received data have certain significance in the scheme of safety evaluation and individual assessment of DRM having acceptable biocompatibility for specific patient.

RESUMO

In experiments on mice immunomodulating activity of root canal pastes (Septodent, Foredent and Sealite Regular) was studied concerning immune response to model allergen. In order to study immunomodulating effect of root canal pastes the authors have used the model of standardized immunization by model allergen ovalbumin (OA) of mice (CBA x C57BL/6)F1. OA (Sigma) was used as model allergen in all experiments both for mice immunization and for testing anti-OA IgE- and IgG-antibodies in reaction of passive skin anaphylaxis in rats and in reaction ELISA. It was established that the tested pastes led to changes in antibody formation dynamics of the IgE- and IgG-classes, at that some of them produced immunosuppression. In that way root pastes could weaken immune protection increasing risk of different diseases rise.

The influence of combined use of gel Metrogil Denta and tooth-paste Mexidol Dent aktiv on processes of peroxide oxidation of lipids and anti-oxidative activity of mouth fluid in treatment of chronic gingivitis in young persons was investigated. The corrective influence of combination of these products on free-radical processes in mouth fluid was shown.

Investigations were conducted on patients with a parodontitis in whome a low-intensive broad band electromagnetic radiation of the HFF-range was used in complex treatment. The authors confirmed that the EMR of the physiological intensity HFF leads to improvement of a parodontologic status and can be recommended in complex treatment of a parodontitis.

RESUMO

Four samples of dental materials (DM) estd for root canal filling, Septodent (S), Zinkoxide (Z), Foredent (F) and Cialite Regular (CR), were studied for their ability in vitro to release histamine (HR) employing the computerized fiber glass based histamine release assay using whole blood from allergic patients and healthy donors. It was found that F induced high and S low levels of HR from blood basophils from both allergic and non-allergic subjects. Z and CR did not induce HR from blood basophils similar to the negative control. The HR assay may be a useful method for preliminary investigation of DM employed for root canal fillings for safety as well as for selection of specific DM for individual patients.

RESUMO

Study of psychoemotional and neurological status of 58 patients with glossalgia showed that stress plays the leading role in development of this abnormality. Neurophysiological analysis of electroencephalograms of patients with glossalgia revealed pronounced generalized restructuring of rhythmic activity of the brain. Ultrasonic dopplerography of the tongue vessels and laser Doppler flowmetry of capillary bloodflow in the tongue confirmed the presence of short-term ischemia in the tongue at the moment of painful attack. Use of modern anxiolytics and antidepressants in combination with ozone therapy in combined therapy of patients with glossalgia is validated.
